DNA Submission Form

You will notice a column labelled “BDI#”. BDI#s are unique numbers assigned by the lab to each sample submitted for DNA testing.

NOTE: You will not have a BDI# for animals that have not been submitted previously for DNA testing. For animals who have been submitted for DNA testing previously, you can find their BDI# on your results spreadsheet. In addition, the VBSS Registrar has been entering BDI#s into our Grassroots Registry. You can search for them using the animal's name and/or Tag number.

We recommend filling out this form on a desktop computer.
